An attractive equipment price can be offset by rigging, shielding, software interfaces, service coverage, coil replacement, injector systems, and downtime risk. Require each equipment proposal to show installation, warranty term, annual service cost after warranty, software upgrades, uptime commitment, and end-of-term options.
A center with insufficient cash reserve may open clinically but still fail financially. Capacity, Modality Mix, and Referral Density Drive Margin
A radiology center becomes more profitable when fixed costs are spread over more completed exams without creating overtime, delays, or quality problems. Because a large share of cost is fixed or semi-fixed, the contribution from the next properly authorized scan can be attractive. But that does not mean every additional scan is equally profitable. The cleanest practical rule: improve booked-and-completed utilization before adding another scanner. A second MRI can increase capacity, but it also adds debt, service cost, staffing needs, and construction exposure before it adds volume.
Where Is Break-Even for a Multi-Modality Center?
Break-even is not the point where billed charges equal expenses. It is the level of collected revenue at which contribution from completed exams covers monthly fixed and semi-fixed costs. Because reimbursement and direct cost differ by modality, a weighted contribution margin is more useful than a simple exam count.

The modality mix changes that answer. If the center loses ten MRI scans but gains ten X-rays, exam count is unchanged while revenue falls materially. This is why daily scan volume alone is not a sufficient KPI. The model should break volume into MRI, CT, ultrasound, X-ray, mammography, and any other service line, with separate net collection and direct-cost assumptions.

Accreditation and quality obligations also affect the dashboard. The American College of Radiology offers modality-specific accreditation for MRI, CT, mammography, ultrasound, nuclear medicine, PET, and other services, so image quality, physicist testing, equipment performance, and staff qualifications cannot be separated from financial performance.

A common structure combines founder or investor equity, an equipment loan or lease, a term loan for build-out, and a working-capital line. Simple payback ignores the time value of money and terminal value, so investors should also review debt amortization, return on invested capital, internal rate of return, and downside liquidity.
