What this estimate hides is lease risk. A low-rent second-generation office can still become expensive if bathrooms, door widths, parking, egress, acoustics, and room configuration do not fit the program. The payroll burden also exceeds the advertised hourly wage. In 2026, the employer share of Social Security is 6.2% up to the wage base and the employer Medicare rate is 1.45%, before state unemployment, federal unemployment, workers' compensation, benefits, or paid time off. Separate productive capacity: an employee paid for 40 hours may produce only 25-32 billable hours in a stable week.

Customer acquisition also works differently from ordinary consumer services. Pediatricians, diagnosticians, schools, case managers, parent networks, and payer directories can produce referrals, but a referral is not an admitted and billable case. Track cost per qualified inquiry, assessment conversion, days to authorization, days to first service, and the percentage of authorized hours actually staffed. Break-Even Depends on Billable Utilization, Not Scheduled Hours
A schedule can look full while the business loses money. Families cancel, staff call out, technicians travel between homes, authorizations expire, assessments wait for approval, and claims are corrected after the payroll tied to them has already been paid. Break-even must therefore use collected revenue and fully loaded direct cost, not gross charges or booked appointments.
If fixed costs are $38,000 per month and the blended contribution margin after direct clinical labor is 40%, break-even collected revenue is $95,000 per month. At a 35% margin, the same cost structure needs about $108,600. At 45%, it needs about $84,400.

Contribution margin improves when collected rates rise, route density improves, group attendance increases, claims are cleaner, and paid hours produce more billable units. It falls when wage pressure, overtime, cancellations, travel, low reimbursement, or denied claims grow. The management goal is not maximum billable time at any cost. It is sustainable billable time with proper supervision, documentation, staff retention, and clinically appropriate service.
Supervision capacity places a real ceiling on scale. The current BACB RBT Handbook requires ongoing supervision equal to at least 5% of the hours an RBT spends providing behavior-analytic services each month, along with required real-time contacts and observation. Payer rules may require more clinical oversight. A model that adds technicians without adding professional capacity can fail operationally before it fails mathematically.

Owner income is not revenue, gross profit, or even EBITDA. If the financial model omits a market wage for the owner's labor, it overstates business profit.

In the base case, the owner receives a $9,000 monthly salary for an active clinical and management role plus an estimated $4,700 distribution, or about $164,000 annualized. That result disappears quickly if collected revenue falls 10%, contribution margin drops below 40%, or the company needs to rebuild its cash reserve.
Existing operations should be valued the same way. Normalize the owner's wage, remove one-time expenses, test payer concentration, review aging receivables, verify authorization utilization, and estimate replacement capital. A practice showing $250,000 of accounting profit may have much less transferable cash flow if the owner personally carries a full BCBA caseload, manages every denial, and recruits every technician.

Set dashboard thresholds from actual contracts and local operations, not internet averages. The ranges above are management starting points. In the first six months, review utilization, cancellations, clean claims, and cash weekly. Review contribution by payer and service monthly. Review labor turnover, supervision capacity, referral conversion, and payer concentration quarterly.
Also track leading indicators. Referral inquiries, completed intake packets, scheduled assessments, pending authorizations, accepted offers, technician training completion, and credentialing status predict revenue before the income statement shows it. A financial model, business plan, or operating template is most useful when these operational inputs flow directly into the revenue and cash forecast.
Licensing, Payer, and Workforce Risks Shape the Financial Plan
Compliance is not a side cost. It determines whether the business is allowed to deliver the service, whether the payer recognizes the provider, whether the claim is payable, and whether records survive an audit. Behavior analyst regulation is state-specific. The BACB licensure map shows that many states license or otherwise regulate behavior analysts, but the founder must still check the current state board, Medicaid manual, commercial payer rules, local zoning, business licensing, and any childcare, respite, or home-care requirements triggered by the exact program.

Paper payback often looks faster than real payback because the formula ignores the opening ramp, receivable growth, owner underpayment, loan principal, equipment replacement, denied claims, and the cash reserve needed to survive payroll volatility. A credible base case should not assume immediate full staffing or full utilization. It should show monthly ramp-up by employee, case, payer, and authorization cohort.
For an acquisition, replace startup cost with purchase price plus transaction cost, integration cost, and working capital. Then normalize earnings, verify cash collections against claims, inspect payer contracts and recoupment exposure, and test whether key clinicians stay after closing. The investment is attractive only if the post-close cash flow supports debt, a market salary for the operator, reinvestment, and a reasonable return on the equity at risk.
